INTENT TO COACH FORMS

Intent to Coach

  • All prospective Coaches, Assistant Coaches, and Team Managers, must complete an on-line Intent to Coach Form to be considered for a Coaching position with Take On Sports.

  • If selected to coach a separate Volunteer Background Form must also be completed for all Volunteers over the age of 18.

  • A NEW Intent to Coach Form must be completed for every sport and each new season.  Coaching in one season or sport does not guarantee selection as a coach in subsequent seasons or other sports.
